Practices will want to Save the Date for the November 7 Learning Collaborative: “Primary Care Works: Effective Strategies to Improve Population Health and Patient Activation”. Peter Anderson MD from Team Care Medicine will be one of the featured keynote presenters.
CTC arranged to have NCQA come to Rhode Island to provide training on the 2017 NCQA training. There were 108 people who attended the “Introduction to 2017 NCQA Standards” session on July 17-18 and 93 people who attended the “Advanced Concepts in NCQA” on July 19. In total, CTC was able to provide attendees with a registration cost savings of $132,465.00 as a result of being able to price the
CTC welcomed 26 new practice sites at the July 20th Orientation Program including 14 adult practices and 12 PCMH Kids practices. . New practice sites received an Orientation Resource Manual that they can use to assist them with meeting the Service Delivery Requirements.
At the July Practice Reporting Committee meeting, Pano Yeracaris, MD CTC Chief Clinical Strategist presented the adult CTC primary care practice all cause ED and IP utilization results (all cause ED for the June 2014-15 to June 2015-2016 time period (insert link Claims Data Collection and Analysis)).

After piloting a successful Community Health Teams initiative for more than three years, CTC was well-positioned to apply for a unique grant opportunity with the State to expand the work of Community Health Teams and Screening, Brief Intervention, Referral and Treatment (SBIRT) screening for substance use disorders and mental health disorders in Rhode Island.
