PCMH-Kids and CTC are pleased to announce that all the health plans have agreed to provide continued financial support for PCMH-Kids Cohort 1 practices.
CTC is proud to announce that, with funding from Tufts Health Plan, we have been able to extend the Integrated Behavioral Health (IBH) Practice Facilitator training program. This month, CTC has accepted three more applicants to become certified IBH Practice Facilitators.
The CTC Board of Directors is pleased to announce that it has approved 9 primary care practice sites for participation in the IBH primary care program. This IBH expansion, funded by UnitedHealthcare, will include 30 providers and 70,000+ patients. The IBH program will start in February with an orientation date scheduled for February 28th.
With funding from the State Innovation Model, Rhode Island Quality Institute is contracting with CTC to host an Advance Directive Learning Collaborative for the 10 participating organizations who will be testing the use of "KnowMyHealthRI" (KMHRI) as a registry for ensuring that
Community Health Team (CHT) evaluation efforts are underway.
