Dear Colleagues:

Please plan on attending our Clinical Practice Champion Quarterly Learning Session (Breakfast of Champions). We will have a clinical focus combined with an overview of the CTC program in transition. This is a critical juncture for primary care in Rhode Island and for the CTC program, and your involvement and input is very important.

ALL clinical practice champions and practice leaders should attend.

Agenda and topics will include:

  • Innovation in Practice;
  • OHIC PCMH Results, Care Transformation Advisory, and Alternative Payment Advisory; and
  • Practice Clinical and Operational Champions Discussion
