Nurse Care Manager/Care Coordinator Best Practice Sharing: Assisting "At Risk" Children and Families

Stephanie Terry, DCYF Assessment and Referral Specialist presented at the August NCM/CC Best Practice meeting and provided information on Rhode Island's mandatory reporting requirements and the Family Care Community Partnership (FCCPs) agencies that are part of DCYF's network of prevention-focused providers. FCCP services are available to all families in RI, helping to identify the services and supports which families may need and assist with accessing and navigating those services. Families do not need to be open to DCYF or referred by DCYF to receive services.
