Improving Population Health / Reducing Low-Value Care in Primary Care through Professional Use of Continuous Glucose Monitoring (Pro CGM) 

At the final Learning Collaborative meeting in July, Pharmacy teams reviewed their implementation of Pro CGM in the management of patients with diabetes, with a focus on its impact on patient care, cost-effectiveness, and quality outcomes. The teams shared their challenges and the need for continued sustainability and team standardization of billing for devices. Stephen Kogut, PhD, MBA, RPh from University of RI, presented aggregate data on the program. Over 500 patients agreed to wear Pro CGM devices and because of this intervention, there was a net reduction of 0.943 points in A1c. This project demonstrated the importance of pharmacists, team-based care and the role that Pro CGM can play in patient engagement by providing patients with data and customized health coaching. This 2-year project was made possible through the support and funding from UnitedHealthcare.
