RI Diabetes Health Equity Challenge Wave Two Momentum Session
On March 12, the East Bay Community Action Program and Pawtucket/Central Falls teams reported out on the progress they are making with using the Pathway to Population Health Compass tool to advance the work of understanding the patient population, identifying patients at highest risk that need additional assistance, increasing the impact of people with lived experience and implementing community solutions. The WE Team has assisted the teams with developing team charters to help articulate their short- and long-term goals, action plans and timelines. To aid in Rhode Island envisioning a future state, the WE Team asked Lena Hatchett PhD, Executive Lead for Proviso Partners in Health to share her reflections on the transformational system change that has occurred in the Proviso communities of Maywood, Bellwood, Broadview, and Melrose Park (“Building a Sustainable Food Economy”). Proviso Partners for Health (PP4H) is a community-driven, multi-sector coalition that has successfully implemented community health, health equity, policy, systems, and environmental change strategies that promote racial, social, and economic equity within Proviso Township.
