CTC-RI is thrilled to announce our Annual Conference “Advancing Primary Care: Innovating for a Sustainable Future” which will be held on November 19, 2025, at the Crowne Plaza in Warwick, RI.  This event brings together our primary care community and other key stakeholders and provides an important opportunity to discuss practical and strategic solutions on topics that are important to you, your team, your system of care and other innovators in our state.

CALL FOR ABSTRACTS AND POSTERS: Due April 11, 2025

We invite primary care team members, systems of care, health plans, community-based organizations and other interested parties to submit abstracts and poster sessions by April 11, 2025. Key themes include innovation in primary care/health care/ population health, (family-child health, aging and older adult), innovations in team-based care (integrated behavioral health, pharmacy, nurse care managers, community health workers), advances in workforce development, and other topics such as patient voice, policy and use of technology.  

With over 300 expected attendees, the conference provides a unique opportunity to explore solutions and innovative practices for improving patient care in primary care and health care across the state. View the full call for applications, requirements, and more.
