Clinical Practice Champion Quarterly Learning Session

On September 11, 2015, the Care Transformation Collaborative of Rhode Island held it's first quarterly Clinical Practice Champion Learning Session.

At this session, there were three presentations:

  • (0:40) Dr. Pano Yeracaris, Co-Director at CTC-RI, provided an introduction to the origination of this series and a plan for moving forward.
  • (31:40) Dr. David Bourassa (Thundermist Health Centers) presented on high risk management strategies.
  • (52:35) Dr. Ed McGookin, of Coastal Medical, presented on their strategy for the reorganization of care coordination.

These series will continue to be held quarterly, for all CTC-RI Clinical Practice Champions.
