CTC-RI/PCMH Kids Hosts Practice Reporting / Transformation Committee Meeting with Salute to PCMH Kids Cohort 3 Practices
PCMH Kids Cohort 3 practice performance was highlighted at the May Practice Reporting and Transformation meeting including the clinical quality measures and CAHPS customer experience survey results. Practices shared results from their performance improvement plans, which focused on improving adolescent well-child visits, team huddles, screening for lead and anxiety. The Data and Evaluation Committee reviewed PCMH Kids Cohort 3 final performance results also in May and approved incentive payments for the practices that achieved 3 out of 5 clinical measures and successful participation in the CAHPS survey.
Congratulations to the PCMH Kids Cohort 3 practices that have successfully completed the 3-year contract effective June 30.
We extend special appreciation to Andrea Galgay, RIPCPC Chief Operating Officer, and Sarah Fessler, EBCAP Chief Medical Officer who have served as co-chairs of this committee and provided significant guidance over many years.
Future Practice Reporting / Transformation meetings will be scheduled on an as-needed basis.
