Care Transformation Collaborative of Rhode Island’s Performance Reporting Portal

CTC-RI and the pediatric patient-centered medical homes (PCMH-Kids) practices are excited to be partnering with Onpoint Health Data to support the transformation of primary care in the state of Rhode Island through analysis and reform efforts designed to improve the quality of life, patient experience, affordability of care, and overall health.

Over the past several weeks, CTC-RI reached out to all of the current and graduated practices in an effort to identify users who should be granted permission to access CTC-RI’s Performance Reporting Portal (PRP) – a secure, web-based reporting platform powered by Onpoint Health Data that delivers personalized views into the health and performance of CTC-RI practice populations.

Drawing on data from the Rhode Island All-Payer Claims Database (APCD), known also as HealthFacts RI, the CTC-RI PRP’s online interface is flexibly designed to fill critical information gaps and address the need for greater transparency into the cost, quality, and utilization of services delivered across our region. Onpoint’s objective is to assist the practices in better identifying variations and gaps in care to help inform care management and improvement decisions.

With an increasing need to put actionable information into the hands of key stakeholders of the CTC-RI and PCMH-Kids initiative, core functionality of the portal allows users to customize dashboard visualizations, drill down into interconnected summary-level measurement screens, and manage all relevant inputs to the CTC-RI-specific master provider directory.
