CTC to provide Practice Facilitation Services for Rhode Island Chronic Care Collaborative and Care + Community + Equity Practices

Effective November 1, 2017, the Rhode Island Department of Health (RIDOH) will be contracting with CTC to provide practice facilitation services for the primary care practices that are participating in the RI Chronic Care Collaborative (RICCC) and Care+Community+Equity (CCE) programs. RIDOH has been a leader in improving population health in Rhode Island and was a pioneer in health care quality improvement work. 

RIDOH initiatives for this contract will focus on the areas of hypertension, diabetes and pre-diabetes,  self-care management and growth in the area of community-clinical linkages. CTC and RIDOH will work together to align efforts to create efficiencies for the primary care practices that participate in both CTC and the RICCC/CCE initiatives. 

CTC will utilize and leverage its existing contract with MHRI to provide practice facilitation services since many of the RICCC and CCE practice sites have working relationships with these practice facilitators. Practices will additionally have access to the larger CTC practice transformation services that assist practices with practicing within patient centered medical home model of care.
