Rhode to Equity at Camden Coalition Conference

Linda Cabral, MM, CTC-RI Senior Program Manager recently had the unique opportunity to attend the Camden Coalition’s annual conference, Putting Care at the Center and to share the work of the Rhode to Equity (R2E) project. The conference serves as an intersectional platform, welcoming consumers, providers, administrators, researchers, policymakers, caregivers, students, and others who are working to improve care for people with complex health and social needs.  

On behalf of the entire R2E team, Linda presented a poster which shared the background of the project, information about teams and highlights to date, as well as partnerships and year 1 outcomes. The poster also featured the Pathways to Population Health framework from the Institute for Healthcare Improvement (IHI) which serves as the foundation for the Rhode to Equity community health teams and year two action plans.
