Kickoff Learning Collaborative: Delivering Age-Friendly Care Implementing the 4M Model and Supporting Care Partners of People with Dementia
This month, CTC-RI successfully launched the “Delivering Age-Friendly Care” learning collaborative, designed to support practices in implementing the 4M Model and advancing care for older adults and individuals living with dementia. The session brought together six participating practices to embark on this six-month quality improvement journey. This initiative, led by CTC-RI in collaboration with RIDOH and Rhode Island Geriatric Education Center (RIGEC), provides funding to six practices as they implement or expand their approach to age-friendly care 4M Framework. Practices are to pursue Level 1 or Level 2 Recognition from the Institute for Healthcare Improvement (IHI). In addition to implementing the 4M framework, participating practices will work on identifying and supporting care partners for patients with dementia, ensuring these caregivers receive proper education and referrals to additional resources. We were honored to have Jenny Knecht-Fredo, a subject matter expert from Penn State College of Nursing, present on the topic, "What Does Age-Friendly Mean to You?" Her insights added depth to our conversation, helping participants reflect on the importance of age-friendly care in their own settings. Participating practices include:
