Nurse Care Manager/Care Coordinator Best Practice Sharing Meeting Updates

In July, 39 nurse care managers/care coordinators (NCM/CC) completed the Glearn core curriculum training program which provides learners with a blended learning experience including robust, proven online modular content, coupled with regularly scheduled Zoom meetings with other learners interested in applying the patient-centered medical home concepts within the Rhode Island environment. NCM faculty, specifically trained in the Glearn curriculum, supported this learning experience with evaluation results indicating increased care management patient-centered medical home skills and abilities. As part of the final requirements for the Glearn core curriculum program, 18 NCM/CCs presented on their capstone care management case studies at the July NCM/CC Best Practice Sharing Meeting. Their poignant stories underscored the clear value of the NCM/CC in the lives of our patients/families during the COVID-19 pandemic as many patients faced added strain and difficulties with management of chronic illnesses, anxiety, depression and isolation.
