Addressing Social Determinants of Health in Rhode Island - Unite Us

On June 18, Account Manager, Emma Romey, presented at the SBIRT/CHT Executive Committee meeting about the Unite Us E-Referral System. Unite Rhode Island connects community partners (such as social service organizations, government agencies, and health care providers) to deliver integrated whole person care through a shared technology platform (Unite Us) to address social determinants of health in Rhode Island. The goal was for the group to establish familiarity with the initiative and demonstrate the core functionality of the system.
