Rhode to Equity Teams Apply Change Management Principles
The 6 Rhode to Equity teams came together this month to learn about change management using the SWITCH framework. The discussion, facilitated by the Wellbeing and Equity in the World Team (WE Team) focused on the 3 steps of the framework: “Direct the rider,” “Motivate the elephant” and “Shape the path.” After receiving a brief overview of the framework, attendees were invited to apply the principles to an issue in their own lives or work.
The conversation continued a week later at the Rhode to Equity coaching meetings, where teams spent the majority of their times in breakout groups applying change management principles to their work so far in addressing health inequities within their communities.
The Rhode to Equity project kicked off in July 2021 and will move into its 2nd year this summer. The project is funded through the RI EOHHS and RIDOH. The effort centers around the Pathways to Population Health framework, bringing together 6 teams made up of a health equity zone (HEZ) as well as other partners in the work: accountable entities, primary care clinics, community health teams, community-based organizations, and individuals with lived experience of inequities.
