Nurse Care Manager/Care Coordinator Meeting: Sharing Best Practices for Meeting Patient Needs During COVID-19
CTC-RI used its Nurse Care Manager/Care Coordinator meeting forum to discuss Pediatric and Adult COVID-19 Best Practices: Screening, telephone triage, telemedicine workflow, getting tested / getting results, and patient resources.
Prior to the meeting, NCMs/CCs completed a survey on what would be helpful to them and what best practices had been put into place within their primary care practice settings. Many thanks are extended to Jennifer Nappi (Women's Medicine Collaborative), Linda Salvatori (Massasoit Internal Medicine), Serena Guibeau (Northern Rhode Island Pediatrics), and Beverly McNally (Quality Behavioral Health) for sharing their protocols and expertise.
Brian Miller from Rhode Island Quality Institute provided an update on important work that RIQI has done to provide timely information to medical community on COVID-19 test results; Elizabeth Lubera, URI student in Cyber-Senior program, provided tutorials on assisting patients with setting up Gmail accounts and using FaceTime; and Maureen Maigret shared information on Project HELLO, an important resource for socially isolated older adults.
