CTC Hosts 3rd Advance Directive Collaborative Meeting: "Medicare's Annual Wellness Visits: A Gateway to Quality"
Susan Whittaker, CPC, CPMA, Healthcentric Advisor Senior Quality Program Coordinator, was the featured content expert speaker at the 3rd Advance Directive Collaborative meeting held on April 18th for the 9 practices/organizations participating in the State Innovation Model (SIM) funded initiative. Susan provided information on how practices can maximize the effectiveness of Annual Wellness Visit (AWV) for patients and staff, increase screening services and quality reporting for screening measures and understand billing and coding for AWVs. Practices can use the AWV as an opportunity to discuss advance care planning with patients and bill for providing this service. Participating practices/organizations reported out on their AIM statements and performance improvement work plans which are being used to increase practice usage of uploading advance directive documents to the consumer engagement platform, KnowMyHealthRI.
The Learning Collaborative sessions are open to other primary care practices/organizations with the next meeting being held on Wednesday June 19th at Rhode Island Quality Institute (7:30-9:00am). Barb Resnick, PhD, CRNP, FAAN, FAANP, professor of nursing at the University of Maryland and past president of the American Geriatrics Society, will be discussing opportunities and challenges for Medicare's Wellness Visit (Part 2).
