RI Dept. of Health Selects Community Health Teams for Expansion Pilot Program

RIDOH is partnering with CTC-RI to provide project management support and practice facilitation services to support two community health teams (CHTs), including South County Health/Wood River and East Bay Community Action Program, in a pilot program aimed at improving the clinical outcomes for patients with hypertension, diabetes and/or at high risk of cardiovascular disease. Community health workers will complete the CVD/DM Specialty training offered by Rhode Island College and then provide health coaching for identified high-risk patients. CHTs, together with selected primary care practices, participated in an orientation kick-off meeting on March 26.
