CTC-RI Hosts 4th Advance Directive Learning Collaborative Network Meeting
Barb Resnick, PhD, CRNP, FAAN, FAANP, professor of nursing at the University of Maryland and past president of the American Geriatrics Society was the featured content expert speaker at the 4th Advance Directive Collaborative meeting held on 6/19/19 for the 9 practices/organizations that are participating in the State Innovation Model (Model) funded initiative. Practice nurse care managers were also invited to attend the learning session. In her presentation “Annual Wellness Visit: Implementation in Real World Settings”, Dr. Resnick discussed the operational barriers associated with Annual Wellness Visits (AWV) and described strategies to overcome these barriers. She additionally shared with the group an Annual Wellness Visit template that she has successfully integrated into her electronic health record. Practices can use the AWV as an opportunity to discuss advance care planning with patients and bill for providing this service.
The Rhode Island Quality Institute provided updated information on enhancements that have been made to the Consumer Engagement Platform “KnowMyHealthRI” and indicated that there is continued funding to support the KMYRI platform. The practices were recognized for piloting the work of uploading advance directives to the consumer engagement platform and providing feedback that is being used by the State in their efforts to improve communication among providers.
