CTC practices highlighted at International Conference on Practice Facilitation
Suzanne Herzberg PhD, MS, OTR/L, Director of Transformation at Brown Primary Care Initiative, presented at the International Conference on Practice Facilitation Tampa, FL in December. Her presentation looked at how practices can use NCQA as a framework to improving care delivery, beginning with staff communication and structure, and pointed to two CTC sites as examples. Providence Community Health Center, a federally qualified health center, has revolutionized its eight practice sites, instituting team huddles and monthly staff meetings that regularly conduct PDSA cycles. Barrington Family Medicine, a micro-practice, has incorporated a nurse care manager into its structure, allowing a focus on population health management and high risk patients. The two practices demonstrate the potential for improvement, despite great differences in size, resources and patient population.
