About Me

Michelle joined the Care Transformation Collaborative of Rhode Island (CTC-RI) in 2022 as a Program Coordinator and now serves as a Program Manager. Throughout her time at CTC-RI, she has worked on initiatives involving the integration of Community Health Workers (CHWs) into multi-disciplinary care teams, strengthening medical/legal partnership efforts, and advancing health equity across Rhode Island. In her current role, Michelle manages projects focused on Autism and Developmental Delay, Children and Youth with Special Health Care Needs (CYSHCN), Asthma and Tobacco Use, CHWs as part of the DULCE Team, and CTC-RI’s annual conference. She enjoys working with state partners, community organizations, and health care leaders to enhance integrated, family-centered care and advance health equity through addressing social determinants of health.

Michelle holds a Master’s in Public Administration with a minor in Nonprofit Management from Johnson & Wales University. Before joining CTC-RI, she served as a Lead Case Management Coordinator at Equus Workforce Services, working directly with families made vulnerable by systemic inequities. She has also served on the boards of Trinity Gateway Association and SHIP and is a former fellow of the Women’s Fund of Rhode Island Policy Institute of the 2019-2020 cohort. Michelle was recognized in 2019 by the Association of Fundraising Professionals Rhode Island Chapter (AFP-RI) as a Partner in Philanthropy.

The mission of CTC-RI is to support the continuing transformation of primary care in Rhode Island as the foundation of an ever-improving integrated, accessible, affordable, and equitable health care system.  CTC-RI brings together critical stakeholders to implement, evaluate and spread effective multi-payer models to deliver, pay for and sustain high-quality, comprehensive, accountable primary care.
