Nurse Care Manager/Care Coordinator Best Practice Sharing Committee Meeting Updates

Important updates were shared at the NCM/CC Best Practice Committee meeting including information on the Enhanced Community Health Team Model to assist primary care practices (pediatric/adult/family) with serving individuals and families who are considered at "high" or "rising" health risk due to significant social and/or behavioral health needs. Expansion includes a larger geographic reach and increased supports that will be available through the Rhode Island Parent Information Network (RIPIN) Pilot. RIPIN has created a new Family Care Liaison position. This person will work directly with practices and families to connect them with: 

  • School supports from RIPIN special education experts
  • Enhanced care coordination services through Cedar Family Center 
  • Assistance if at-risk for food and housing social needs

Nurse care managers and care coordinators are encouraged to call 401-384-7831 when they encounter families that have needs and may be unsure of how to help them access needed services.
