The CTC-RI Board of Directors is pleased to announce that it has approved 9 pediatric primary care practice sites to participate in the Pediatric IBH primary care program. This IBH expansion, funded by Rhode Island Foundation, Tufts Health Plan and UnitedHealthcare, serves ~40,000 kids and an average Medicaid population of 52% with some sites as high as 80-90% Medicaid.
On May 30th, the American Academy of Pediatrics RI Chapter hosted the final meeting for the PCMH Kids Adolescent Screening, Brief Intervention and Referral to Treatment (SBIRT) Learning Collaborative at the Warren Alpert Medical School. This learning collaborative would not have been possible without the support and funding from Tufts Health Plan and the AAP Healthy Children Grant. Drs.
CTC-RI hosted a quarterly meeting with PCMH Kids practices and key stakeholders on June 6th. Dr. Flanagan reviewed practice incentive results. 15 out of 20 PCMH Kids practices met 3 out of 4 quality measure thresholds and are eligible for incentive payments from the health plans. 100% of PCMH Kids practices met Developmental Screening targets. In addition, Dr.
PCMH Kids practices will be eligible to receive multi-payer sustainability payments when they complete the PCMH Kids contract. Medicaid has confirmed that it is including PCMH Kids sustainability payments in the managed care rates for FY2020.
RI World Diabetes Day (RIWDD) was launched in 2019 by the RI Department of Health as a vehicle for raising awareness about diabetes and its impact on residents. Medication-Assisted Treatment (MAT) is the use of FDA-approved medications, in combination with counseling and behavioral therapies, to provide a "whole-patient" approach to the treatment of substance use disorders. In an effort to ensure CTC-RI Practices are on track to obtain sustainability payments with the Office of the Health Insurance Commissioner when eligible, CTC-RI now offers practices an "OHIC Targets" tab to view their quarterly clinical quality data against OHIC's thresholds. For Q1 and Q2 2019, practices can view measures data displays of both Adult Cohort 4 and 5; and PCMH Kids Cohort 2.

In January 2015, 25 adult practices representing 90,000 patients and 100 providers entered into the Care Transformation Collaborative of Rhode Island program as "Cohort 4" with the goal of improving quality, customer experience and reducing costs.
The mission of the Care Transformation Collaborative of Rhode Island is to lead the transformation of primary care in Rhode Island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve.
