CTC-RI and the pediatric patient-centered medical homes (PCMH-Kids) practices are excited to be partnering with Onpoint Health Data to support the transformation of primary care in the state of Rhode Island through analysis and reform efforts designed to improve the quality of life, patient experience, affordability of care, and overall health.
We are grateful for the ongoing work of our board of directors, staff, primary care team members, behavioral health clinicians, community health workers, health plan partners, and state partners for their work to make 2017 a year of progress for our Collaborative.

The Team-Based Care Learning Collaborative is a 10-month participatory learning experience offered by the National Cooperative Agreement for Clinical Workforce Development, funded by the Health Resources and Services Administration, and hosted by Community Health Center, Inc.
The mission of the Care Transformation Collaborative of Rhode Island is to lead the transformation of primary care in Rhode Island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve.
